My Doh moment was when I pulled into the Sami station and disconnected the elect plug from the tv to avoid drain my tv battery while dumping the tanks and forgetting to hook it back up. I noticed the brake controller flashing while travelling down the mountain and sure enough, no trailer brakes. Fortunately my truck was capable of stopping it and I pulled over as soon as I could.

Modern TV's have a relay that usually disconnects TV from truck battery when the truck is off, FYI.
I have twice forgotten to latch the hitch (!) and had it flop a bit making odd noise/feeling and then I remember, stop and latch it.

Wasn't my d'oh but my dealerships. Picked up our new trailer and they forgot to secure the propane tank. 20 minutes later a car behinds me is honking frantically as the tank started dragging on the road with sparks coming off of it.

Latest d'oh was picking up new trailer. Really two d'oh! together. I forgot a bracket so had no anti-sway, dealer had left 80 gallon fresh water tank 2/3 full. Drove over 100 miles until I figured-out why I could not get over 50mph without white knuckle sway.
Years ago had a small PuP disconnect at the ball, luckily the crossed chains worked but back of truck was torn up from the tongue while stopping. I now I triple check and always have pin in the ball lock.

Got the new TT home and started to back into the driveway without removing the sway bar. Stopped mid turn just in time!

Re-read the thread title. Oh, the BIGGEST d'oh!
Let's see...
...neglecting basic maintenance and having to replace wheel bearings on the side of Interstate 81 on a Sunday afternoon. Then having to replace the whole axle.
...knowingly locked the keys inside the car?
...driving drowsy and totally destroying the car? No injuries, but blocked the road for several hours.
...using an angle grinder without guards or gloves? Scars on both index fingers for that *repeated* stupidity.
...letting a Diesel tractor run out of fuel in the middle of a poison ivy patch?
...(professionally) not bothering with a full backup before updating 5,200 union member records (its just 1 data field). Because of overlapping field names I slammed their home address to that of their home local. 5 hours spent cleaning up THAT mess.
...grabbing hold of the test lead that was connected to the BOOST driver of a color TV set I was fixing: 400VAC @ .5A 15Khz. Had the shakes for the rest of the day.

Well....it was supposed to be an RV-specific d'oh moment but I've consulted with the judges and YOU WIN!!

My biggest "d'oh" was pulling the pin on the trailer to keep it from rolling backwards in an unlevel site and then wondering why the generator was needed to try to bring the battery back to a good charge state. When I put the pin back in, the generator got the battery charged back up much more easily. Good thing we didn't stay away from the FW any longer than we did. Hate to think how the FW would have traveled down the site before just digging in and stopping after bending the front jacks beyond usability. That one was dumb as a post.

I never make mistakes, I'm just demonstrating what happens when others make mistakes. I think of it as a teaching opportunity for the DW and kids.

Believing our first dealer that our tt was a true four season tt and finding out that every thing was frozen that first cold morning of our trip
Yikes I was scared it was going to be an expensive ordeal
My pinnical 5 th wheel will be ready for the cold this winter or it won't be going out

Funny DOH moment over memorial day. Pulling out after hitching up, thought we did the whole check and were good to go. Get one block and hubby realized he forgot the towing mirrors. So he gets out and pulls them out of the camper garage and puts them on (We have the molded ones that slide over the mirror) and ff we go. Stuck in traffic on the freeway at a standstill some nice guy waves us down. Tells us the garage on the camper is open When Hubby got the tow mirrors out her forgot to close it back up. We went 20 miles before someone told us. Luckily I organize the garage with bins and stuff so nothing fell out. My favorite part was right after we hitched up and it was going smoothly I told hubby we were doing pretty well for N00Bs. He had replied he didn't think we were N00Bs anymore that we had the hang of it. yup then he did that.
