+1, you need it to be on the ground so you can safely and effectively put the leverage on the torque wrench without spinning the wheel or pulling it off the jack you've got it on. Just like when you're taking it off, you break the lugs before you lift, and torque while it's on the ground.

Not unless there was some unusual circumstance- like the lugs were so loose that the entire wheel was loose too.

The important thing to remember is that lug nuts DO have to be tightened in a particular sequence (pattern) and, naturally, that sequence differs depending on whether your wheels have 4, 5 or 6 lug nuts.
