I believe you'd just call it the shower skylight.

It's a skylight and you would probably want the exact same size.
Measure what you have and here is a place to start....

No idea what it costs, but if you're handy it should be easy to replace. Remove the Dicor caulking around it. Remove the screws, and lift it out. Put the new one in and put new Dicor self leveling caulk around it. Whee removing the Dicor be careful not to tear the rubber roof.

I use a plastic putty knife about 1 1/2 in. wide.. Be very careful and remove it until you can access and remove the screws. Notice how it is sealed and replace the new one the same way,
Caulking well or using Eternabond tape.

Be very careful pulling it up as it is sealed top and bottom.

The seal is usually butyl (putty) tape, but I think some might use a foam seal. After the new skylight cover is screwed in place (evenly and not so tight it cracks the rim of the skylight), slather on the Dicor sealant and you'll be good. I had to replace mine, and the hardest/most tedious part was removing the old sealant. I ordered the cover from Amazon, at around $90 bucks.
