I ended up downloading the InfiLog app from iTunes. It is completely customizable to whatever you want to keep track of. You can create drop down lists, slider bars, star ratings, manual entry fields, graphs, check boxes, etc. You can also export. They also have a free version to try out before you buy. Fit the bill for me!

I made one using Excel. I keep track of dates, miles, the CG's we stay in with a 5star rating and a column for notes. It also has running totals for nights and miles and yearly totals for the same. It was free, does just what I want and when I think of another feature, I can add it.
Recently I added a running total of miles on the new-ish tires.

My wife and I use a different method to document our RV excursions. We buy a refrigerator magnet from each place we visit and put it on the door of the RV fridge.
Simple, but effective.
