Problem with slide

We recently noticed our slide started making a loud scraping sound when we opened or closed it. My husband crawled under it a took a couple pictures. The actuator is no longer centered in the hole and is scraping on the side of the hole. Has anyone ever had this type of problem and knows what would cause this to happen suddenly? Any help is appreciated.

I haven't seen what your pictures show, however I wonder if the frame is in a bit of a twist. If you are using stabilizer or leveling jacks then I would suggest to test again without them in place and see if that makes any difference. Also, when closed, does the slide appear to be properly in position or off a little bit? ~CA

I would take some measurements to see if the slide opens evenly from the side of the trailer to the outside of the slide when opened.

When opened is the inside flush to the wall?

I do see a lot of rust!
I would go underneath and clean the moving parts and observe the slide when opening.

X2 this area seems to have been overlooked for it regular maintenance and dry lube for the gear track. Wire brush that tract and apply a dry lube preferably twice a year.
