Not really a request for help, but more of a "have you seen this before?" post.
Let me fill you in on what is going on... Furnace wouldn't start up last year, found it was the blower motor that was bad. Replaced it. But still had some issues with it, I would have to cycle it a couple times to get it to start. But it was the end of the season, and that's an issue for next fall Squeaky.
Well, next fall is here, time to get it working. Replaced the sail switch and limit switch, still nothing. Would turn on, run for 4 minutes and 5 seconds, and shut off. Called Suburban, they gave me the manual and how to fix it.
It started to look like an issue with the fuel, electrode or burner. I figured since it was under warranty, to go ahead and give our service guy a call.
He comes out, pulls the furnace, and finds the burner is disformed, along with a portion of the burner chamber.

He said he's never seen that before and will ask around what the cause could have been.
We have a solution and fix in progress, my question to the community of experts, is what could have caused this, and how to avoid it in the future?
