I always put a piece of buytle tape on the back side of the gutter extension, then use a screwdriver to gently lift the gutter forward, slide the gutter in being careful not to roll the tape, then anchor with the screw. This will ensure there’s no water intrusion if the gutter gets plugged at the extension.

The screws are covered with sealant pretty good. I don’t want to cut/scrape the sealant if I don’t have to to find the screws :-)

You can replace the sealant, don’t let that intimidate you, using the screw to anchor the extension ensures the extension doesn’t blow off during your drive.

This should at least be in your decision matrix. I wish I had known this existed prior to installing mine. I may have still done it the way I did, but I like to have more options available to choose from.
One advantage of the ones that stick to the overhead cab is that the water is diverted out away from the doors. I had a problem with the water dribbling down the side of the rig. My extensions solved that problem, but now I have a stream of water coming out of the extensions right in my path of ingress to the driver's seat. I can avoid it, but it would be cool if it weren't there at all...
As for installation (I installed something similar to what you're looking at), I agree with the previous posters in that you shouldn't be afraid to remove a little sealant to get to the screw. This was one of my primary concerns when I did mine, so I had a tube of OSI Quad on hand and sealed it back up all nice and neat after I installed the gutter extension.

Ha... you beat me too it...
I bought the gutter extensions the OP linked, and ended up throwing them out. They would have helped with the black streak situation, but wouldn't help get the water away from the doors.
The biggest issue was on the passenger side, where not only did the rain water or A/C draining run right in front of the cab door, it also splashed down by the coach entry door. If the campsites was dirt, it made a big mud puddle and splashed mud right by the coach entry door. Running the gutter extension to the front of the cabover eliminated both of those issues.

I am too, unfortunately it makes quite a mess on the front tire if you are parked on dirt or crushed rock. Still worth it though, definitely.
