Does the pilot stay on all the time or just as needed

In a word, no...in another word, yes. What hot water heater do you have? My 2012 Jayflight SLX 185RB did. My 2002 Cherokee 5r doesn't, nor my previous Salem Cruise lite241QBXL, nor my current Jayfeather 23RL. Tell is more...

If your water heater is dsi Direct Spark Ignition it has an electronic ignition that lights it on demand.
You can hear it click when outside trailer and water heater is on and needs heat.

If you have a rocker switch to turn it on/off, most leave it on. I have met some that turn it off until they plan on using hot water and then just wait for it to heat up.

Most water heaters have an electronic ignition that lights the pilot and it stays on until you turn the water heater off. My current camper has an on demand tankless water heater that does not have a pilot. When you turn the hot water on the heater ignites the burner and controls the flame to hold the water temperature at the set temperature.

Dizz55 it is always a good idea or just absolutely necessary to include your RV information when asking a question like this.

Some water heaters like on my 195RB SLX have pilot lights that you have to light outside manually. Others have pilot lights that are lighted by spark igniters, some just ignite with a spark.

Many also have an electric heating element that you can turn on instead of using propane when you have electricity.

You can add your tow vehicle and RV info on your signature line which saves repeating it.
