The Blue Ox comes with an adapter for the ball so you can use one with either a 1" or 1 1/4" ball shank. I preferred to not use the adapter so since I was buying a new ball I just bought one with a 1 1/4" shank. I probably didn't need one rated for 20,000# but it won't hurt. It was a little more expensive than the ones rated to 10-14K but it wasn't a huge difference.
I think I mentioned this before but I set the hitch roughly up myself by measuring the coupler height with the camper leveled. Then added an inch to that to come up with how high to set the hitch. I didn't torque anything down though and the dealer put the parts on the A frame, verified the measurements (not sure if they moved anything) then torqued the ball, hitch and other parts down. We were in the dealer's store when they were doing that so I didn't see the entire process.
