Power Cord Lights are ON

Just plugged my surge protector into the shore power and all lights are green. I have an extension cord added to my factory power cord. All cords are the same brand and AMP but the factory cord that came with the travel trailer has a red and green indicator light and both lights are ON. What does this mean? Everything is working fine in the camper.

Red usually indicates reverse polarity, which is a problem.

I would start by eliminating parts. Can you eliminate the extension cord to test?
