Patch Identification

I purchased a 2004 Jayco Qwest 10B a few years ago. It came with a few patches in the canvas that held up very well over the last few years. However, due to this years extreme heat, they've started to fail. I've purchased a couple cheaper brand patches on Amazon, but none last more than a couple days. I'm wondering if anyone can identify the patch I've included a picture of. Both the patch, and adhesive used would help greatly. I'd like to re-apply a similar patch that will last a few years and doesn't need to be replaced every few weeks. There is a patch on the inside and outside for each hole. Thank you!
